INSTRUCTIONS: 

WORD FILES
"Word" links are Microsoft Word documents. To read these documents, you must have Microsoft Word 97 or higher 

ACROBAT FILES
Adobe Acrobat reader is available for free by clicking on this link.  The Acrobat Reader allows you to fill out the forms and print them, but you cannot save the completed forms on your computer unless you have Adobe Acrobat Professional or higher.

FOXIT
If you would like the ability to fill in the Acrobat files AND save them (filled out) on your computer, you can download the free Foxit reader by clicking here.

.ZIP FILES
To reduce file size and improve download times, all documents have been compressed into .zip files.

To use these .zip files you need a program that can extract the individual files contained within them. There are several different programs out there, but here are a couple of options:

• WINZIP - The most popular, feature-rich and easy-to use program is WinZip. WinZip is not free, but they do have a 21-day trial version.

• 7-ZIP - The program is not as user-friendly, but it gets the job done and it is FREE.
